Registered Nurse Mental Health

Join the Adult Community Mental Health Team in Southwest Victoria and enjoy a rewarding coastal lifestyle while providing essential mental health support to adults living with moderate to severe mental health concerns.

In this role, you’ll manage a community caseload, delivering assessment, treatment, and recovery-focused case management. You’ll provide both in-office and outreach support, including crisis and risk assessments, development of recovery plans, coordination of referrals and close collaboration with consumers, carers and local services to promote wellbeing and recovery.

As part of a well-structured multidisciplinary team including nurses, psychologists, social workers and occupational therapists you’ll have the opportunity to work in a supportive environment that values trauma-informed and recovery-oriented practice.

Key Responsibilities:

• Provide comprehensive mental health assessments and ongoing case management
• Deliver outreach and home-based support to consumers
• Develop and review individual recovery and treatment plans
• Work collaboratively within a multidisciplinary team
• Coordinate referrals and liaise with community and health services
• Maintain accurate clinical documentation and uphold professional standards

About You:

• Registered Psychiatric Nurse (RPN3), Occupational Therapist, Psychologist, or Social Worker
• Current AHPRA registration (or relevant professional membership)
• Minimum 5 years’ experience in mental health clinical practice
• Demonstrated skills in crisis assessment, intervention, and recovery-focused care
• Excellent communication and teamwork skills
• Current driver’s licence essential
